Mission & Vision
The S.A.L.T. – School to Adult Life Transition Series™ was created and organized to guide families of individuals with disabilities ages 14 - 22+ through the difficult and confusing transition from school to adulthood. We are dedicated to provide transition related training opportunities along with helpful resources for families to reference at their convenience. Through our various programs, we strive to create an easier transition for individuals with disabilities and their families as they continue their journey into adulthood.
Our SALT Leaders Team
The members of our SALT Leaders Team consists of several representatives from the local county board of developmental disabilites. Each one of our leaders brings a unique set of skills and expertise to our programs.

What is SALT Resource Solutions LLC?
Our Story
Patti Shrom is a mom, wife, entrepreneur and self-starter. Personal life experiences and the decisions faced as each stage of the school to adult life transition presented proved to be very challenging and confusing.
As her family worked through each step of these stages, from school to adulthood, eligibility, services, employment & beyond, left her feeling frustrated and alone. She was determined to help other families navigate this process by offering training guidance and resources so each parent can become empowered and a
strong advocate for their child.
S.A.L.T. Resource Solutions, LLC. was created as a consulting organization with a mission to help establish additional S.A.L.T. - School to Adult Life Transition Series™ programs for families in multiple areas. By collaborating and consulting with local area county boards of developmental disabilities services, agencies or organizations, S.A.L.T. Resource Solutions, LLC hopes to bring valuable resources and valuable contacts to families, caregivers or staff involved with individuals that have disabilities.
With focus on family and building towards independence, S.A.L.T. Resource Solutions, LLC is unique and a valuable resource to help families reach these goals.
